PET REGISTRATION

Please fill out the information below to register your pet with Unique Apartment Homes. All information must be filled out and complete to get your pet registered BEFORE the pet has moved into the unit. Tenants must provide a current picture of the pet and a valid rabies vaccine/certificate. The rabies must cover the full duration of your lease. If the rabies expires before your lease ends, you must provide management with the updated vaccine report at or before the time of expiration. After management receives the submission and approves the pet, tenants will receive an addendum through their online portal for the pet listed below.

By submitting this form it does not guarantee your pet will be approved. Management reserves the right to refuse any pet/animal for any reason – breed, species, size, location of the apartment, etc.

• Pet Rent: $50/month
• One Time Fee: $250 non-refundable
